INFORMATION PROVIDED TO THE DATA SUBJECT FOR THE PROCESSING OF PERSONAL DATA

In accordance with art. 13 of D.Lgv. 196/2003 (T. U), and in relation to personal data of which Italiana Group s.r.l. will receive, We inform you of the following:

1. PURPOSE OF DATA PROCESSING.

The treatment that will be requested or acquired for personal data is exclusively on the implementation of aims, by Italian Group s.r.l., in the performance of professional activity relating to the evaluation, treatment and definition of out-of-court settlements, concerning the accident case we take charge.

2. DATA PROCESSING METHODS.

a) the treatment is performed by means of transactions or series of transactions listed in article 4 paragraph 1 lit. a) TU: collection, recording, organization, storage, consultation, processing, modification, selection, extraction, comparison, use, interconnection, blocking, communication, cancellation and destruction of data.

b) operations can be performed with or without the aid of electronic or automated means.

c) treatment is done by the owner and/or data controllers.

3. PROVISION OF DATA.

Providing personal and sensitive data is strictly necessary and means provided for the purposes of carrying out the activities of paragraph 1.

4. REFUSAL TO PROVIDE DATA.

The refusal by the individual to give public and/or sensitive personal data implies the impossibility of proper execution of the mandate of paragraph 1.

5. COMMUNICATION OF DATA.

Personal data may be disclosed to those responsible for treatment and may be disclosed, for the purposes of paragraph 1, to stakeholders such as adjusters and insurance companies, experts and coroners, service companies that are entrusted with the management and/or the liquidation of the procedure, delegate to the payment banks, external databases and all those persons to whom the communication is necessary for the proper carrying out of the purposes of paragraph 1 and of the mandate received.

6. DATA DISSEMINATION.

Personal information cannot be disclosed for purposes other than the given mandate.

7. DATA TRANSFER ABROAD.

Personal data may be transferred to EU countries and to countries outside the European Union in the framework of the purposes of paragraph 1.

8. RIGHTS OF THE DATA SUBJECT.

Article 7 T.U. confers specific rights, including the right to obtain from the Holder the confirmation of the existence or otherwise of their personal data and making them available in an intelligible form; the stakeholder has the right to know the source of the data, the purposes and methods of processing, of the logic applied to the processing, the identity of the owner and of the persons to whom the data may be disclosed; the stakeholder has the right to obtain updating, rectification and integration of information, its cancellation, transformation into anonymous form or blocking of data processed unlawfully; the holder has the right to oppose, for legitimate reasons, the processing of data.

9. THE DATA CONTROLLER.

GENERAL CONSENT TO THE PROCESSING OF PERSONAL AND SENSITIVE DATA

The customer and/or User of services offered by Italiana Group s.r.l. declares that:

Having read the above information and the fact that submitting required common, personal, health and sensitive information is necessary to start the procedure of claim, I expressly guarantee my consent in accordance with articles 23, 24, 25 of the aforementioned legislative decree to treatment (including collection, recording, organization, storage, processing, modification, selection, extraction, comparison, use, interconnection, blocking, communication, cancellation and destruction) of my personal data by the persons referred to in the abovementioned legislation and within its limits.

I maintain however all rights provided by law and in particular the right of access, rectification and deletion of data, wherever legitimate.
